PassportMD, Inc., a privately held company headquartered in South Jordan, Utah, is a leading provider in the consumer-directed healthcare and personal health records (PHR) industry. Remaining committed to our goal of "Independent Family Wellness™," PassportMD helps individuals keep track of all facets of their family's health and wellness including personal health records and documentation, doctor communication, pharmacy and expenses.

Our company was founded by a physician who saw the vital need to create an easier approach for time-strapped individuals who want to manage their own healthcare and that of their family. Since its launch in 2004, PassportMD has received numerous awards for our software application and was most recently voted "Best of Class" and recognized as one of the top PHR systems on the market as it relates to "data, data integrity and consumer control".

Medicare and CMS have chosen PassportMD as one of only four vendors to participate in the PHR pilot program for Medicare.

Financial Disclosure

With PassportMD, you may sign up on our annual plan. We make money by selling memberships selling our MediConnect Services, products through our store, by providing custom private label websites using the PassportMD technology, through our customer's use of our MyRemindeRx service and our customers utilizing our PassportMDRxCard. The fact that certain products and services are sold through our store is not an endorsement of those products or services and does not influence the editorial content on our site. In our storefront, we have a Wallet size CD-ROM and a Key Chain USB where customers may order to store their health information. This information is easily uploaded to either of these devices. PassportMD does not sell any drugs. We are not a pharmacy nor do we endorse or recommend any medication.
